Duration of sick leave
The reasons for receiving sick leave can be different: illness, pregnancy, surgery, caring for a relative or child, etc. Each base has its own duration.
On average, treatment of the disease takes from three to fifteen days. The maximum period that a doctor has the right to give without collecting a commission is one month. If the patient does not recover during this period, a medical commission meets and decides on the advisability of an extension. If the commission sees positive dynamics, which in the long term will lead to the patient’s recovery, it can extend the BL for up to a year.
If the period fixed by the doctor turns out to be really short, the commission may extend it until the disability is restored or until the disability group is established.

Treatment can be carried out in outpatient, inpatient, or sanatorium-resort settings. Sick leave certificates are also issued for the period of pregnancy and childbirth, for caring for children and sick relatives, as well as for adoption.
On average, outpatient treatment lasts 2-10 days. It could be ARVI, flu or poisoning. If symptoms do not disappear during this time, the period is extended to a month. In such cases, there is no need to assemble a medical commission.
Reference! If the patient lives in a place where there are no hospitals nearby, and he has to travel a long time to get to a medical facility, then travel time should be taken into account when calculating the period.
In the inpatient method, that is, treating a sick person in a hospital, a certificate is issued for the entire period of the person’s stay in the hospital. After discharge, the doctor may give 10 days for rehabilitation. Days are provided only if the person requires outpatient recovery.
If treatment in a sanatorium-resort setting is necessary, the period will be equal to the duration of the trip, including travel there and back.
The maximum period of sick leave to avoid being fired due to illness
Federal Law 255 states that sick leave is allowed for 6 months. In practice, citizens always try to hand over the paper to the employer the next day after discharge. There are situations when, for some reason, an employee submits a document later, but not exceeding the permissible six-month period. In this case, the employer will be obliged to pay for days of forced absence from the workplace.
A certificate of incapacity for work is issued by the attending doctor when, based on the test results, a diagnosis is established and the appropriate prescription is made. During the period of diagnosis, the doctor has the right to give a sick leave with a minimum validity period of 3–5 days.
When is it renewed?
There are several good reasons for extending sick leave:
- identification of complications;
- long recovery of the patient;
- acquisition of injury during illness;
- prescribing to a patient during treatment of a surgical operation;
- the document was received to care for a person who has developed complications of the disease or whose body takes a long time to recover;
- the occurrence of complications during pregnancy and childbirth.
Any other reasons cannot be grounds for extending the BC.
Basically, if it comes to several days without involving a medical commission, then doctors have no problem extending it if the patient is weak. However, they cannot extend it without reason.
Certificates are mainly issued by state clinics, since they all have licenses. Not everyone who is private wants to undergo full certification, so they do not issue certificates.
If sick leave is extended for more than 15 days, the patient must be seen by a doctor for examination and diagnosis of his current condition.

Maximum period of sick leave in 2021
The main document confirming temporary disability is a sick leave certificate (SL). The algorithm for issuing BC is regulated by Order of the Ministry of Health and Social Development of the Russian Federation dated June 29, 2021 No. 624n. The minimum period of sick leave is not established by law and, as a rule, is determined by the attending physician. According to Art. 11 of Order No. 624n, the doctor has the right to individually issue a sick leave, the maximum duration of which cannot exceed 15 days.

If after 15 days the patient has not recovered, then the attending physician sends him to undergo a medical examination, which determines the need to extend the BL. If the patient was treated in a private licensed clinic, then after 15 days the doctor will also refer him to the medical institution at the place of registration (residence) so that the issue of extending the BC can be decided by a commission of doctors (Article 13 of Order No. 624n). If you need to continue sick leave, you need to follow some important rules. The patient must undergo an additional medical examination before the commission. The survey date must be given five days before the meeting. If the patient refuses to undergo the commission, the sheet is closed immediately without the right to extension.
The patient must constantly visit the clinic to assess the condition of the disease and the dynamics of recovery. Missing one such visit may serve as grounds for closing the sick leave.
Maximum duration of sick leave: what it depends on and how it is extended
The possible timing of such an extension is stipulated in Federal Law No. 323 dated November 21, 2021. It states that standard sick leave can become longer by a maximum of 15 more working days, and if the sheet was originally issued by a paramedic or dentist, then only by 10 days. And in any case, the patient will have to undergo a medical examination.
Note! After the first two weeks of recovery, the patient will have to attend every meeting of the medical commission so that specialists can observe the process of his recovery (in various stages) and objectively record the current condition.
How many times and for how long
You can extend your sick leave with a therapist only once; repeated extensions are possible only when the issue is discussed by a medical commission. If the period approved by the medical commission has also expired, the sick person is sent for a medical examination, where the dynamics of recovery are monitored.
If positive dynamics are present, then the sick leave is extended until the patient fully recovers; if not, a working or non-working disability group is awarded. In the latter case, the sick leave is closed upon receipt of the results of the medical examination.

How many days is sick leave given?
- If the duration of illness is more than 30 days, then in addition to the signature of the attending physician, there must be a signature of a VC specialist.

Separately, we note the day hospital, in which a person does not stay overnight in the hospital. However, he needs to come there every day to undergo procedures, such as an IV or an injection. In this case, a sick leave certificate is issued for the entire period of such therapy. Therefore, if a person needs 10 IVs, then he will not have to go to work for 10 days.

The therapist can extend sick leave for up to 30 days and only once . All other actions must be approved by a medical commission.
Medical commission
When extending sick leave for more than 15 days, decisions are required to be made by a special selected medical commission. Any verdict of theirs is recorded in the table “Exemption from work” with two signatures.
In what cases can sick leave be extended?
For diseases that are not classified as serious, they give from 3 to 15 days to improve their health . This is the period for which the doctor issues a certificate of incapacity for work. If recovery does not occur during this period, in agreement with the medical commission, the bulletin is extended for another 15 days. You can “ballot” longer in the following cases:
Repeated extension of sick leave by decision of the medical commission is possible, but in very rare cases. As a rule, after the expiration of the established period, a decision is made either to close the sick leave or to send it to MSEC.
Order
The patient comes to the local clinic for examination. Usually the doctor opens a sick leave on the day of treatment. If the patient is unable to come, you can call a doctor at home. The doctor chooses the treatment method: inpatient or outpatient.
For the first time, a doctor can open sick leave for no more than 15 days. If the patient needs more time to recover, the doctor's stay can be extended for another 15 days with the consent of the head of the department.
Further, all actions related to the extension of sick leave are carried out only when the test sheet is reviewed by a medical commission. In this case, the patient himself must consent to the medical examination. These requirements must be observed by doctors in any situation and in any institution that provides medical care.
Important! For treatment in a hospital, no maximum period is established - a sheet is issued for the entire period of treatment.
If the sick employee was treated in a hospital and also underwent surgery, it is clear that after being discharged from the hospital he will need more time to recover and strengthen the body. To avoid problems, every person needs to know how certificates of incapacity for work are extended after surgery. In this situation, all issues are resolved with the attending physician personally without communicating with the medical commission.
He can add days to sick leave himself in the following cases:
- The patient requires additional measures to improve health (15 days can be added).
- After inpatient treatment, the patient needs outpatient treatment to consolidate the result. This is plus 10 days to the deadline.
- If unforeseen complications arise, another 10 days may be added.
A maximum period of sick leave of 10 months is given if the patient has undergone a labor-intensive operation, the person has suffered a heart attack or stroke, and the patient has a complex fracture.
How many days is sick leave given without VK?
If a child under 14 years of age is ill, sick leave is issued for up to 15 days to a family member who will care for him. Depending on the severity of the disease, it is extended for no more than 10 days. If the disease requires a hospital stay, sick leave is issued for the entire period of the child’s treatment there.

A sick leave certificate for the duration of the operation is issued for the period from the day of admission to the hospital until the day of discharge from it. The duration of stay on sick leave after surgery is 10 days. The commission may extend this period by 10-12 months in case of complications or the need for long-term postoperative therapy.
Extension due to pregnancy
Sick leave for pregnancy and childbirth also has its own terms. The duration may vary depending on the course of labor and the number of children born.

For how many days is sick leave issued?
Not only the person who is sick himself can receive sick leave, but also the person who is caring for a sick minor child or relative, and is also on maternity leave.
If the patient has not recovered within the allotted time, the doctor may extend the sick leave for another 30 days. If the patient does not recover during this time, then only a medical commission can extend his sick leave for a maximum of 10 months, subject to positive dynamics in the course of the disease.
